Integrated Care Models: Supporting Youth's Holistic Needs
Dr. Angela Diaz and Joey Martinez hosted a Learning Session for past and present Upswing grant recipients on September 16, 2022 about integrated health care models.
Integrated care models are marked by comprehensive, inter-disciplinary, and patient-centered approaches to health care and well-being. In the presentation above, Joey provides a detailed overview of the history of integrated care and benefits to youth mental health.
Angela goes on to share how integrated care models help to close equity gaps in service to diverse communities and provides both patient and provider examples of this model in action at the Mount Sinai Adolescent Health Center.
